      1711.A.&B.&D.&E.: Based on observations at 11:30 a.m., S1 failed to ensure child to staff ratio was met at all times as the Specialist observed S1 and S2 supervising 15 children aged 1-year-old to 4-years-old. They could only supervise 14 children. There needed to be 1 additional staff present. This could not be corrected prior to the Specialist's departure. Corrective Action: Effective 04/16/2026, S1 stated she has hired an additional staff member, but is awaiting the CCCBC-based determination of eligibility.       Based on record review at 12:00 p.m., S1 failed to ensure at least two staff members are trained in medication administration. This could not be corrected prior to the Specialist's departure.
