1711.A.& B.& D.&E.: Based on observations at 9:15 am, S1 failed to meet the required child to staff ratio for children of the following age: infants. The Specialist observed six children, ages five months to one-year-old, in the infant classroom. The required ratio for children of this age is 5 to 1. S1 corrected the ratio issue, prior to specialist departure. Corrective Action: Effective 2/9/2026, S5 stated she will create spreadsheet with children's classroom assignments and place in classrooms, to ensure compliance with this regulation.

1713.A.&B.&C.: Based on observations, at 9:15 a.m., S4 failed to supervise children at all times in the center. The Specialist observed S4 exit the infant classroom, leaving five children, ages five months to one-year-old, unsupervised with the classroom door closed, while S4 stood in the doorway of S3âs classroom to ask a question. The Specialist observed the infants remain without direct supervision for 1 and a half minutes. Corrective Action: Effective 2/9/2026, S5 stated she will have a floater, available each day, in the center, to ensure compliance with this regulation.

1901.J.&K.: Based on observation at 9:15 a.m., S1 failed to ensure items that can be harmful to children, were kept in a locked cabinet or other secure place that ensures they are inaccessible to children. The Specialist observed one mop bucket, with water and a mop in it, sitting on the floor in an open eating/classroom area, accessible to children. Corrective Action: Effective 2/9/2026, S5 stated she will provide a training for all staff, which will cover hazardous regulations and placement of hazardous items in the center. to ensure compliance with this regulation.

Based on observation at 9:15 a.m., S4 failed to ensure awake children did not remain in a crib, for more than 30 consecutive minutes. The Specialist observed 3 infants standing in a crib. At 9:32 a.m., the Specialist re-entered the infant classroom and observed the same three still standing in their cribs. Review of the daily infant reports indicated the infants had been changed at 9:02 a.m. and S4 stated they were placed into their cribs. S4 did not remove the infants from the cribs. Based on observation at 9:15 a.m., S4 failed to ensure infants were held and were not placed lying down on a mat or otherwise with a bottle or sippy cup. The Specialist observed one infant in bouncer with a bottle, propped to the side. Corrective Action: Effective 2/9/2026, S5 stated she will re-train staff on infant room regulations and guidelines, to ensure compliance with this regulation.
