Based on observations at 12:30 p.m., S1 failed to ensure the outdoor areas as the Specialist observed broken plastic at the bottom of the white slide on the playground where children play. This was not corrected prior to the Specialist departure. Corrective Action: Effective 10/3/2025, S1 stated she will have the slide removed from the playground by 10/10/2025 to ensure compliance with this inspection.
