      Based on observation at 11:50 a.m., S1 failed to ensure the outdoor area was free of hazards. The Specialist observed the wooden seat on the seesaw split in half which can cause cuts, bruises and the children to smash their fingers. This was not corrected prior to the Specialist departure. Corrective Action: Effective 10/23/2025, S1 stated she will contact the church maintenance department to have the wooden seat fixed and will not allow any children to play on the equipment to ensure compliance with this regulation.

      Based on observation at 11:15 a.m., S3 and S4 failed to ensure the manufacturer's restraint device was used when the feeding table was occupied by children. The Specialist observed C1, 1-years-old, sitting unrestrained in a seat at the feeding table. S3 stated the strap was missing. This was not corrected prior to the Specialist departure. Corrective Action: Effective 10/23/2025, S1 stated that she will order new straps for the feeding table and no child will be allowed to sit in that seat to ensure compliance with this regulation.

      Based on observation at 1:50 p.m., S1 failed to ensure the center's bus had a current inspection sticker. The current inspection sticker expired 07/2025. This was not corrected prior to the Specialist departure. Corrective Action: Effective 10/23/2025, S1 stated she will take the bus to get a current inspection sticker and will perform a check monthly for expiration dates to esnure compliance with this regulation.

Based on record review and interviews at 12:00 p.m., S1 failed to notify within 24 hours of the incident the Department of the following critical incident: On 02/17/2025 at 8:17 a.m., O1 reported to S1 that S3 (DOH 09/15/2020) grabbed C1, 3-years-old, too hard and pulled C2's, 3-years-old, hair and bumped into him causing him to fall down. S1 completed the Critical Incident Report (CIR) during the inspection on 03/24/2025 and submitted it to the Department.
