The last End of Day Visual Check was completed on 05/05/2026. Corrective Action: Effective 06/26/2026, S1 stated that the End Of Day Check Visual Check will be completed by S1 each day and that she will continue to monitor it daily to ensure compliance with this regulation.

Based on Record Review/Interview on 8/28/2025, at 11:00 a.m., S1 failed to obtain a CCCBC-based determination of eligibility for S10. S10 was present at the center on 8/11/2025, 8/12/2025, 8/14/2025, 8/15/2025 and 8/19/2025, performing child care duties without an eligible background check. Corrective Action: Effective 8/28/2025, S1 stated she will obtain CCCBC-based determination of eligibility for all new hires before placing in classrooms, to ensure compliance with this regulation.

Continuing Education Training - Based on record review/interviews at 01:00 p.m., S1 failed to provide documentation that all staff who are left alone with children, or who have supervisory or disciplinary authority over children, have obtained a minimum of 12 clock hours of continuing education per center anniversary year. S9 (DOH 08/31/2023) failed to obtain the minimum requirements of 12 clock hours required for the 2024 anniversary year. S9 obtained 9.5 hours.

Mandated Reporter: Based on record review/interview at 01:00 p.m., S1 failed to ensure all staff members completed the Mandated Reporter training annually. 3 of 14 staff members, S5, S7, and S10, did not have current certification. S1 stated each staff member will have proof of Mandated Reporter Training by 01/22/2025.

Based on record review and interviews at 11:30 a.m., S1 failed to provide documentation that 5 of 11 staff completed the online child abuse and neglect Mandated Reporter Training provided by DCFS annually. S1 provided training certificates for S1 and S4, however, their training was expired. S2, DOH 9/13/2024, S6, DOH 10/16/2024, and S9, DOH 9/13/2024, are new hires.

Based on record review and interviews at 11 a.m., S1 failed to document daily that the entire center and play yard is checked after the last child departs to ensure that no child is left unattended at the center. The end-of-day checks have not been completed since 10/18/2024.
