      Based on observation, interview, the licensee did not comply with the section cited above in that there is a 10in. hole on the outside wall near the ramp that leads to the entrance of toddler classroom, a post with chipped paint outside of the preschool classroom and areas without a barrier that have access to an unused bigger playground which poses a potential health, safety or personal rights risk to persons in care.
