Based on observations at 12:15 p.m., S5 and S6's classrooms failed to be lighted in such a way as to allow visual supervision of the children at all times. This was corrected in S6's classroom. Corrective Action: Effective 10/22/2025, S1 stated she will install additional light sources in S5 and S6's classrooms to allow visual supervision of the children during nap, to ensure compliance with this regulation.

Based on observations at 12:10 p.m., cribs failed to be free of toys or other soft or loose bedding while the child was in the crib, as an infant was observed sleeping in a crib with a loose blanket draped over them. This was corrected prior to the specialist leaving the classroom. Corrective Action: Effective 10/22/2025, S1 stated she will retrain staff at the November meeting on making sure cribs remain free of all items, including blankets and toys, to ensure understanding and compliance with this regulation.

Based on observations at 12:10 p.m., written authorization from a physician failed to be available for 2 of 7 infants as required for an infant to sleep in a car seat or other similar device (rocker; swing). The infants were removed from the devices and transferred to their cribs prior to the specialist leaving the classroom.
