1103.A.3.&C.2.: Based on record review and interviews at 2:30 p.m., S1 failed to notify DCFS within 24 hours of the following critical incident: On 9/2/2025, at 10:31 a.m., C1 (3-years-old) was left alone on the play yard for approximately 3 minutes by S10, who failed to complete a headcount before the class returned inside after playing. 1713.A.&B.: Based on record review and interviews at 1 p.m., children failed to be under supervision at all times as on 9/2/2025, at 10:31 a.m., C1 (3-years-old) was left alone on the play yard for approximately 3 minutes by S10, who failed to complete a headcount before the class returned inside after playing. Based on record review and observations at 12:30 p.m., the center's staff and owner's daily attendance record did not accurately reflect persons on the child care premises at any given time as S1 failed to sign out when she left the center on break and then back in once she returned. Corrective Action: Effective 10/22/20205, S1 stated she will add a reminder sign near the exit door to clock out for breaks to ensure compliance with this regulation.

1711.A.&D.: Based on observations at 12:10 a.m., child to staff ratio failed to be met for the following group of children: 7 infants with 1 staff, S3. The required ratio for children of this age is 5 children per 1 staff person. At 12:12 p.m., S4 joined S3, satisfying ratio requirements. Corrective Action: Effective 10/22/2025, S1 stated she will retrain staff at the November meeting on child to staff ratio requirements, to ensure understanding and compliance with this regulation.

Based on observations at 12:15 p.m., S5 and S6's classrooms failed to be lighted in such a way as to allow visual supervision of the children at all times. This was corrected in S6's classroom. Corrective Action: Effective 10/22/2025, S1 stated she will install additional light sources in S5 and S6's classrooms to allow visual supervision of the children during nap, to ensure compliance with this regulation.

Based on observations at 12:10 p.m., cribs failed to be free of toys or other soft or loose bedding while the child was in the crib, as an infant was observed sleeping in a crib with a loose blanket draped over them. This was corrected prior to the specialist leaving the classroom. Corrective Action: Effective 10/22/2025, S1 stated she will retrain staff at the November meeting on making sure cribs remain free of all items, including blankets and toys, to ensure understanding and compliance with this regulation.

Based on observations at 12:10 p.m., written authorization from a physician failed to be available for 2 of 7 infants as required for an infant to sleep in a car seat or other similar device (rocker; swing). The infants were removed from the devices and transferred to their cribs prior to the specialist leaving the classroom. Corrective Action: Effective 10/22/2025, S1 stated she will retrain staff at the November meeting on removing infants from devices as soon as they fall asleep and transferring them to their cribs, to ensure understanding and compliance with this regulation,
