1725.B.&C.: Based on record review at 1:30 p.m., S1 failed to ensure at least two staff members were trained in medication administration whether the early learning center administers medication or not. 0 of 8 staff had a current certification. Corrective Action: Effective 6/03/2026, S1 stated she will schedule a medication administration training for all staff by 7/31/2026 to ensure compliance with this regulation.

Based on record review at 2 p.m., S1 failed to document that the entire center and play yard was checked after the last child departed to ensure that no child was left unattended at the center. The last documented visual check of the center was on 3/2/2026. Corrective Action: Effective 3/4/2026, S1 stated she will instruct opening staff to check behind her each morning to ensure proper documentation was completed for the visual check log the day before, and correct as needed, to ensure compliance with this regulation.

Based on record review at 2 p.m., the center's daily attendance record for children failed to accurately reflect the children on the child care premises at any given time as 29 children were present and 25 children were signed in on the log. This was corrected prior to the specialist leaving the center. Corrective Action: Effective 3/4/2026, S1 stated she will be diligent in reminding parents to sign their children in/out appropriately and verify daily that the attendance log is accurate, to ensure compliance with this regulation.

Based on record review at 2 p.m., the center's staff and owner's daily attendance record failed to accurately reflect persons on the child care premises at any given time as 6 staff were present and 4 staff were signed in. This was corrected prior to the specialist leaving the center. Corrective Action: Effective 3/4/2026, S1 stated she will remind staff to sign in/out appropriately and verify daily that the attendance logs are accurate, to ensure compliance with this regulation.

1811.A.&B.: Based on record review at 3 p.m., documentation of a CCCBC-based determination of eligibility failed to be requested and obtained for S4 (DOH: 1/22/2021), at least once during a five-year period. Per the CCCBC website, S4's CCCBC-based determination of eligibility status expired on 1/29/2026, and was renewed on 2/4/2026, with an eligible status. Per center records, S5 continued to work at the center with an expired status from 1/29/2026 - 2/3/2026. Based on record review at 10:45 a.m., S1 failed to have documentation that 3 of 10 staff, S5, S6, and S7, who were on the premises and accessible to the children have a current certification in infant, child, and adult CPR through training approved by the Department. A training is scheduled for 07/24/2025. Corrective Action: Effective 7/9/2025, S1 stated she will compose a spreadsheet with all staffs' certification expiration dates in order to schedule renewal classes timely, to ensure compliance with this regulation.

Based on record review at 10:45 a.m., S1 failed to have documentation that 3 of 10 staff, S5, S6, and S7, who were on the premises and accessible to the children have a current certification in pediatric first aid through training approved by the Department. A training is scheduled for 07/24/2025. Corrective Action: Effective 7/9/2025, S1 stated she will compose a spreadsheet with all staffs' certification expiration dates in order to schedule renewal classes timely, to ensure compliance with this regulation.

Based on record review at 11:00 a.m., S1 failed to ensure that 10 of 10 staff, S1-S10, completed the annual online child abuse and neglect Mandated Reporter Training provided by DCFS in 2024. Corrective Action: Effective 7/9/2025, S1 stated she will compose a spreadsheet with all staffs' certification expiration dates in order to schedule renewal classes timely, to ensure compliance with this regulation.

Based on record review at 11:15 a.m., documentation of a CCCBC-based determination of eligibility failed to be requested and obtained for S6 (DOH: 05/07/2025), at least once during a five-year period. S6's CCCBC expired on 07/15/2024, and she was present in the childcare center from 05/07/2025 to 06/20/2025. S6 received a CCCBC-based determination of eligibility on 6/23/2025. Corrective Action: Effective 7/9/2025, S1 stated going forward she will not allow staff without an eligible CCCBC to be on site or perform services at the center, to ensure compliance with this regulation.

Based on observations at 10 a.m., S1 failed to have bottle formula/breast milk for infants labeled with the child's name for 6 of 7 bottles. Corrective Action: Effective 7/9/2025, S1 stated she will retrain infant room staff on checking bottles upon receipt from parents to verify they are labeled, to ensure compliance with this regulation.

Based on record review at 11:45 p.m., S1 failed to have documentation of tornado drills that were conducted at least once per month during the months of March, April, May, and June 2025. Corrective Action: Effective 7/9/2025, S1 stated she will set a calendar reminder for next year to begin completing tornado drills in March (through June) and maintain documentation for each drill, to ensure compliance with this regulation.
