      1509.A.8.a.&b.: Based on record review/interview at approximately 1:05 p.m., despite the center developing and implementing a written behavior management plan, S10 used a prohibited method of discipline as a child was subject to physical punishment and verbal abuse. On 10/14/2025, between 8:54 a.m. and 10:51 a.m., S10 was observed by O2, during a CLASS observation, pulling on C10's, 3-years-old, arm and telling him to shut up while the class was participating in a painting activity.       1713.A.&B.&.C.: Based on observation at approximately 4:15 p.m., S13, failed to ensure children were under supervision at all times. S13 left C11,11-months-old, in the Director’s office alone, which resulted in C11 hitting his head on a desk. S13 returned to the room when she began to hear C1 scream. S1 stated S13 received a verbal warning for the incident. Corrective Action: Effective 4/10/2026, S1 stated she will retrain all staff on the importance of ensuring children are supervised at all times to ensure compliance with regulation.

      Based on observation at approximately 2:55 p.m., S1's transportation arrangements failed to conform to state laws, as both center van's vehicle registration was expired. The vehicle registration had not been valid since 9/30/2024. This could not be corrected during the center inspection.
