Personnel records or copies of records were not being maintained at the facility and available for review by the licensing authority. CCF Handbook, Section 7.4 Record Keeping [SR] LS reviewed all ten personnel records. The new staff's (listed on the Supplemental Sheet) file was not on the premises during this inspection. However, her valid background screening was pulled up and presented to this LS. She is not CCP. She cleans and cooks. Technical support was provided. 7.4 Personnel Records Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: A. A complete employment application with the required statement pursuant to Section 402.3055(1)(b), F.S. B. Documentation of position and date of employment. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel.

The facility's transportation log did not include [a second signature]. CCF Handbook, Section 2.5.2 (Section 6.2, number 3) General Requirements [SR] The driver verified that a second sweep was conducted but the staff forgot to sign the Transportation log (on three different occasions). The staff signed the Transporation Log today. Technical support was provided. 2.5.2 Transportation Log A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). Transportation By Vehicle E. Prior to transporting children by vehicle, the transportation log must be recorded with each childs name, the date and time of departure and initialed by child care personnel verifying that all children were accounted for and that the log is complete. F. Upon arrival at the destination by vehicle, the driver of the vehicle must: 1. Mark each child off the log as the child departs the vehicle; 2. Conduct a physical inspection and visual sweep of the vehicle interior to ensure that no child is left in the vehic…

The play equipment was not maintained in a safe or sanitary condition for the children to use in that [three pedals were missing on the trike round and the sections were loose]. CCF Handbook, Section 3.12, B and F Sanitation and Equipment [SR] The trike round was removed from the playground during this inspection. Technical support provided. B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. F. All equipment used in the outdoor play area must be constructed and maintained according to manufacturers recommendations and allow for water drainage. Any open containers with water must be emptied immediately after use, i.e. pots, toys, or other equipment that collects water.
