      Based on record review, the licensee did not comply with the section cited above due to 5 out of 5 child records were missing infant 15-min sleep logs which poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 10/17/2025

      Based on observation the facility did not comply with the section cited above due to having sleep sacks in all four cribs which poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 10/17/2025
