      Based on record review, the licensee did not comply with the section cited above due to 5 out of 5 child records were missing infant 15-min sleep logs which poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 10/17/2025

    • Infant Care Activities (C) An Infant Shall Not Be Swaddled While in Care. This Requirement Is Not Met as Evidenced By:CCR 101430(a)(3)(C)

      Based on observation the facility did not comply with the section cited above due to having sleep sacks in all four cribs which poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 10/17/2025

This requirement is not met as evidenced by: Based on record reviews, evidence, and interviews an unqualified staff (S3) has provided care and supervision to children in the infant room alone without having fully qualified teacher or Site Director, who was supervising the next door in preschool room on numerous occassions.
