      will be submitted to the Department by 3/20/23. 8 9 10 11 12 13 14 LPA observed two carpets in the toddler room that need to be cleaned. LPA observed a storage room with stuffed animals and sheets in which LPA was unsure if they were clean. This is a potential risk the health and safety risk. 8 9 10 11 12 13 14

      to the Department. The director will have the walls repaired by 3/24/23 and submit a plan of correction to the Department. 8 9 10 11 12 13 14 LPA observed the carpet lifted in the preschool classroom which needs to be tacked down. In addition, LPA observed a wall with chipped corking and paint.
