      A refrigerator was observed without a thermometer or with a thermometer showing a temperature reading greater than 41 degrees Fahrenheit. CCF Handbook, Section 3.9.2, F.1. (Section 2.1 Health and Safety, Page 3) Food and Nutrition [SR] During the inspection the refrigerator temperature was reading 60 degrees and was full of food. Technical Assistance from Handbook given: 3.9.2 Food Storage Proper storage of food is essential to prevent food contamination, as well as, insect and rodent infestation. Correct handling and storage of all food is a key component School-Age Child Care Facility Handbook | P a g e 29 in preventing food-borne illnesses. To prevent bacteria growth, cold food must be kept at or below 41 degrees Fahrenheit and hot foods at or above 135 degrees Fahrenheit. This was fixed at time of inspection by the staff who threw away all the milk and perishable items. During the inspection a staff member from the YMCA brought over milk to meet all components for supper. To make sure this doesn't happen again the staff members are going to check the thermometer once the refrigerator was empty and if the temperature is still running high then a new refrigerator will be ordered for the site and in the meantime the provider will food delivered daily to serve.       Milk and/or food sat out longer than 15 minutes prior to the beginning of the meal. CCF Handbook, Section 3.9.5, C (Section 2.1 Health and Safety, Page 3) Food and Nutrition [SR] During the inspection Licensing Specialist observed milk in a crate that was sitting on a table and when asked what they were doing with the milk provider stated that it had been delivered today and when asked how long it had been sitting out the provider stated at least 30 minutes. Provider was informed that milk can't sit out for longer than 15 minutes. This was corrected at time of inspection when provider poured out the milk in the sink. Technical Assistance given from handbook: C. Milk and food must not sit out for longer than 15 minutes prior to the beginning of the meal to avoid contamination and spoilage. Food must not be served to children at more than 110 degrees Fahrenheit. Allow time for food to cool before serving to children that does not exceed 15 minutes.

      The facility's menu was not [ was not posted at the beginning of each week in an easily seen place, was not written]. CCF Handbook, Section 3.9.3, H (Section 2.1 Health and Safety, Page 3) Food and Nutrition [SR] During the inspection the provider did not have a menu posted. When asked provider stated that the "site lead" is the one who posts the menu, but she is out on leave. Technical Assistance given from the handbook: H. Meal and snack menus must be planned, written, dated, and posted at the beginning of each week in an easily seen place, accessible to the parents/guardians. Any menu substitutions must be noted on the menu. A generalized menu of possible snack choices for programs that receive food donations is acceptable. All meals and snacks prepared outside of the facilitys kitchen or designated food preparation area, such as catered food, must be listed along with the source. Daily meal and snack menus must be maintained for a minimum of 12 months for licensing purposes. Operators who participate in the USDA Food Program must keep menus in accordance with the Department of Health and USDA requirements. Provider must submit menu via email by 04/08/2025

      Supervision of children in the [afterschool ] group was inadequate in that [a child swallowed magnets]. (Section 5, numbers 1 - 17) General Requirements [SR] The two teachers in the afterschool program told a child to put the magnets up and even after he didn't the teachers did not take the magnets away and the child swallowed them. Teacher failed to take the child seriously once they were told and teachers failed to notify parents either over the phone or in person at pick up.
