      Milk and/or food sat out longer than 15 minutes prior to the beginning of the meal. CCF Handbook, Section 3.9.5, C (Section 2.1 Health and Safety, Page 3) Food and Nutrition [SR] Licensing arrived in the cafeteria at 3:27pm and the milk was out of the refrigerator being served to the younger children and then staff in charge went outside to make a phone call and when she went to serve the older students milk it was 3:54 which is 27 minutes the milk sat out at room temperature. Provider stated she didn't know that at her last place of employment it could sit out iced for an hour, but licensing said this milk is not sitting on ice. This provider is on the Food Program. This was resolved when provider poured out all of the milk that was sitting out. Due Date Completed at time of inspection

      Employment history check did not include all required information including applicants, job title, description of regular duties, confirmation of employment dates, and level of job performance. CCF Handbook, Section 5.1, D (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Provider did not do a 5-year employment reference check. Provider completed 3 years. Licensing asked staff member if she started working in February 2022 or did, she have jobs before then and provider stated, "no she has had many jobs before then". Provider given an extended time frame to complete the job reference check because the provider is closed from 12/19/2025-01/06/2026. Corrective Action: Provider will email licensing with a completed 5-year employment history verification. Due Date 1/7/2026
