Personnel records or copies of records were not being maintained at the program and available for review by the inspection authority. (Section 19.4) Record Keeping [SR] FSC reviewed 4 personnel records, but the facility had an employee on site working who did not have any paperwork. FSC verified he had a current background screening on file. Please see confidential page. The on-site lead person does not have access to the Clearinghouse website to make sure all staff are listed. Human Resources within the program do all the updating of personnel.
