Personnel records or copies of records were not being maintained at the facility and available for review by the licensing authority. CCF Handbook, Section 7.4 (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Provider did not have a file for employee listed on the supplemental. Provider stated she had been at the facility working since 11/10/25. Employee attempted to reach her supervisor AF, but AF failed to respond to provider while licensing was present. Corrective Action: Provider is to email a copy of employee's file to licensing by 01/07/2026. Provider will be closed from 12/19/2025- 01/06/26.
