      Based on observation, interview and record review, the licensee did not comply with the section cited above. Director stated that the other eight children usually attend YMCA Play - n - Grow #198015299, but due to a physical plant issues at Play-n-Grow, 10 children were relocation to the facility above. This was not reported to the department. which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 09/13/2024

    • Responsibility for Providing Care and Supervision for Infants (B) Staff Shall Physically Check on Sleeping Infant(s) Every 15 Minutes and Document the Following: This Requirement…CCR 101429(a)(2)(B)

      Based on record review, the licensee did not comply with the section cited above: LPA did not observe an infant sleep log for toddlers. The facility is only documenting when children fall asleep and wake up. This poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 09/26/2024
