The Specialist observed (C1,5-month-old) and (C2, 12-month-old), without labeled bottles. The Specialist informed S1 and S2 that bottles should be labeled for all children. Corrective Action: Effective 3/5/2026, S1 stated she will label all bottles upon arrival, to ensure compliance with this regulation. Bottles were labeled prior to Specialists departure.

F.: Based on observations/record review at 9:45 a.m., S1 failed to have documentation of CPR and Pediatric First Aid for S3 within 90 calendar days from the date of hire, and prior to assuming sole responsibility for any children. S3 (DOH 1/06/2025) was observed with sole responsibility of 6, two to three-year-old, children. Corrective Action: Effective 8/04/2025, S1 stated all staff will be trained in Pediatric First Aid and CPR within 90 calendar days from the date of hire, and will be certified by 8/08/2025, to ensure compliance with this regulation.
