Based on record review at 11:00 a.m., S1 failed to provide documentation of End-of-Day-Check. Results including date and signature of Staff member completing check has not been documented. The Specialist observed that the End-of Day-Check had not been completed for March 30-31, of 2026. Corrective Action: Effective 4/1/2026, S1 stated she will inform all staff, to complete a daily center check and make sure End-of-Day-Check documents are completed daily, to ensure compliance with this regulation.

Based on Record Review at 9:30 a.m., S1 failed to maintain a daily attendance record for children which accurately reflect children on the center premises at any given time and the name of the person or entity to whom the child is released. The Specialist observed children not signed out of the center and the name of person whom the child is released was not recorded in the months of February and March of 2026. Based on record review at 9:45 a.m., S1 failed to provide documentation of End-of-Day-Check. Results including date and signature of Staff member completing check has not been documented. The Specialist observed that the End-of Day-Check had not been completed for 2/11/2026, 2/12/2026, 2/13/2026, 2/27/2026, 3/3/2026 and 3/4/2026. Corrective Action: Effective 3/5/2026, S1 stated she will double check End-of Day checks in the evening and make sure forms are accurately documented, to ensure compliance with this regulation.

Based on observation/interview at 9:00 a.m., S1 failed to provide written authorization from a physician for a child to sleep in a car seat. The Specialist observed C1, 5-month-old, infant, sitting in a car seat in the classroom, during the visit. Corrective Action: Effective 3/5/2026, S1 stated she will remove infants from car seats upon arrival and remove car seats from classroom, to ensure compliance with this regulation.

1919.J. Based on observations at 9:00 am, S2 failed to have bottle formula/breast milk for infants labeled with the child's name. The Specialist observed (C1,5-month-old) and (C2, 12-month-old), without labeled bottles. The Specialist informed S1 and S2 that bottles should be labeled for all children. Corrective Action: Effective 3/5/2026, S1 stated she will label all bottles upon arrival, to ensure compliance with this regulation. Bottles were labeled prior to Specialists departure.

F.: Based on observations/record review at 9:45 a.m., S1 failed to have documentation of CPR and Pediatric First Aid for S3 within 90 calendar days from the date of hire, and prior to assuming sole responsibility for any children. S3 (DOH 1/06/2025) was observed with sole responsibility of 6, two to three-year-old, children. Corrective Action: Effective 8/04/2025, S1 stated all staff will be trained in Pediatric First Aid and CPR within 90 calendar days from the date of hire, and will be certified by 8/08/2025, to ensure compliance with this regulation.
