      Based on observation, interview with Director, and record review, the licensee did not comply with the section cited above in 3 staff out of 21 staff. S#5, S#9, and S#10 show "In Process" in Guardian which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 10/11/2021
