Child(ren) did not have a Student Health Examination/DH (Form 3040), or an equivalent health statement on file within 30 days of enrollment. CCF Handbook, Section 7.2, C (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Specialist reviewed all 13 childrens files. 1 files did not have Student Health Examination forms. Assistant called parents to have the child picked up. The child will not be allowed back at school until the forms have been brought in. The facility has until April 30, 2026, to have completed forms in the childrens files. The Child Care Facility Handbook states on page 60: 7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. CCF Handbook, Section 7.1 (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Specialist reviewed all 17 children files. 7 children had outdated immunization records. The director will have all 7 children immunization records prior to May 28, 2025. The Child Care Facility Handbook states on page 59: 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C- 22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health.

The facilitys fencing walls or gate area had gaps that could allow children to exit the outdoor play area. CCF Handbook, Section 3.5, F Physical Environment [SR] Licensing Specialist observed a broken section of fence near the gate. There are also several gaps in along bottom of the fencing. The Child Care Facility Handbook states on page 24: The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or build-up to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. However, if outdoor play area fencing is changed then the standard would apply, and two exits must be provided.

The CCF Handbook states that hazards on the playground pose harm to the children.
