F.: Based on observations/record review/interview at 11:15 a.m., S1 failed to have documentation of certification in Pediatric First Aid and CPR for S2 within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children. S2 (DOH 5/20/2026) was observed with sole responsibility of 8, 3-4 year old children. Corrective Action: Effective 5/26/2026, S1 stated she will have all staff CPR certified prior to assuming responsibilities in the center, to ensure compliance with this regulation.

Based on record review at 9:30 a.m., S1 failed to have documentation on file for independent contractors, which should include person's name, address, phone number, and list of duties performed while at the center. The Specialist observed O2 walk into the center at 9:30 a.m., sign the attendance log and proceed to an infant classroom to provide therapy services alone. The Specialist observed O2, O3, O4 and O5 in a classroom with O1 participating in a training. S1 did not have a CCCBC-based determination of eligibility for O2, O3, O4 and O5. Corrective Action: Effective 3/27/2026, S1 stated she will obtain CCCBC-based determination of eligibility documents for all visitors, to ensure compliance with this regulation.

Based on record review at 11:00 a.m., S1 failed to provide documentation of End-of-Day-Check. Results including date and signature of Staff member completing check has not been documented. The Specialist observed the End-of Day-Check had not been completed for March 24, 25 and 26 of 2026. Corrective Action: Effective 3/27/2026, S1 stated she will move End-of-Day-Check form outside of the office, a visual place for daily reminder, to ensure compliance with this regulation.

1919.J. Based on observations at 9:00 am, S2 and S3 failed to have bottle formula/breast milk for infants labeled with the child's name. The Specialist observed 1 out of 7 infants without labeled bottles. The Specialist informed S2 and S3 that bottles should be labeled for all children. Corrective Action: Effective 3/27/2026, S1 stated all bottles received will be labeled upon entering the center, to ensure compliance with the regulations.

Based on record review at 10:45 a.m., S2 failed to have documentation that 3 of 10 staff on the premises and accessible to children have a current certification in Pediatric First Aid and CPR before assuming sole responsibility for any children. S7 (First day on the premises 8/14/2025), S9 (First Day on the premises 10/6/2025), and S10 (First Day on the premises 10/21/2025) failed to have the current certification.
