F.: Based on observations/record review/interview at 11:15 a.m., S1 failed to have documentation of certification in Pediatric First Aid and CPR for S2 within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children. S2 (DOH 5/20/2026) was observed with sole responsibility of 8, 3-4 year old children. Corrective Action: Effective 5/26/2026, S1 stated she will have all staff CPR certified prior to assuming responsibilities in the center, to ensure compliance with this regulation.

The Specialist observed 1 out of 7 infants without labeled bottles. The Specialist informed S2 and S3 that bottles should be labeled for all children. Corrective Action: Effective 3/27/2026, S1 stated all bottles received will be labeled upon entering the center, to ensure compliance with the regulations.
