Based on record review/interview at 10:45 a.m. on 2/3/2026, S1 failed to have documentation of immediate notification to the parent when the following occurred to a child: An incident occurred on 1/29/2026 at 3:37 p.m., in the infant classroom with S12 and S13. C1, seven-months-old, pulled on a toy, causing it to fall on head leaving a small bruise on his forehead. The parent, O1, was notified at 4:30 p.m., during pick up time.
