Based on record review at 12:30 p.m., S1 failed to have S3's CCCBC-based determination of eligibility available for review on the center's CCCBC roster. S8's date of hire with the agency was 8/8/2024; her first day working at the center was 1/28/2025. This failed to be corrected during the inspection.

Based on record review/interview at 12:30 p.m., a CCCBC-based determination of eligibility for child care purposes from the Department failed to be obtained for each independent contractor, O1 and O2, prior to them being present at the center or performing services. Per center records, O1 and O2 were present at the center on 3/11/2025; neither O1 nor O2's based determination of eligibility was available for review on the center's CCCBC roster. S1 stated she did not have access to the CCCBC site to check O1 and O2's eligibility and confirmed they were not accompanied during their time…

C.: Based on record review/interview on 1/14/2025, at 12 p.m., S1 failed to notify the parents immediately and the Department and DFCS within 24 hours of the following critical incident: On 12/18/2024, around 10:30 a.m., while outside on the play yard, S1 noticed that C1 and C2, both age 3, had their pants pulled down and were exposing their privates to one another. S1 notified both children's parents that evening at pickup about the incident, where O3 accused S1 of failing to supervise the children properly. Based on interviews on 1/14/2025, at 12 p.m., children failed to be under supervision at all times. On 12/18/2024, around 10:30 a.m., there were 25 children, ages 3-to-4-years-old, with three staff, S1, S2, and S3, on the play yard. C1 and C2, both age 3, pulled down their pants and exposed their privates to one another. After noticing, S1 rushed over and pulled the children's pants back up. S2 and S3 failed to notice the incident had occurred. The time C1 and C2 initially pulled down their pants or for how long is unknown.

C.: Based on record review on 1/14/2025, at 11 a.m., S1 lacked documentation that 3 of 6 staff, S1, S4, and S6, completed orientation requirements within seven and thirty days of the first day present at the center and prior to having sole responsibility for any children. S1 failed to complete and have documentation for verification of the LDE Key Training Modules 1-3; S4 and S6 failed to complete and have documentation for verification of the DCFS Mandated Reporter Training. This could not be corrected during the inspection.

Based on record review on 1/14/2025, at 11 a.m., S1 failed to have documentation that 2 of 6 staff on the premises and accessible to children have current certification in infant, child, and adult CPR through training approved by the Department. S2 and S4 failed to have current certification. A class has not been scheduled.

Based on record review/observation on 1/14/2025, at 9:45 a.m., A CCCBC-based determination of eligibility for child care purposes from the Department failed to be obtained for each independent contractor, O4 and O5, prior to them being present at the center or performing services. Upon arrival to the center, the Specialist observed both O4 and O5 performing services with children, unaccompanied by center staff not needed for ratio purposes. This was corrected during the inspection.

Based on observation on 1/14/2025, at 11:30 a.m., S1 allowed children under age 4 to be served food that is implicated in choking incidents. The Specialist observed children eating large chunks of chicken (in gumbo) for lunch. This failed to be corrected during the inspection.
