Based on interviews on 5/16/2025, at 11 a.m., children failed to be under supervision at all times. On 5/13/2025, S6 and S19 brought children, ages 1-2-years-old, inside from the playground and did not conduct a headcount. A parent arrived to pick up her children around 4:20 p.m., and saw a child playing in the side play yard. She parked her vehicle, went over to the fence, and saw it was her son, alone and unsupervised. She informed two staff, S6 and S19, that her child was alone on the playground.
