Based on record review on 5/28/2026, at 10:20 a.m., S2 failed to ensure the center's staff and owner's daily attendance record accurately reflected persons on the child care premises at any given time as 3 of 22 staff were not signed in or out on the log, as appropriate. S16 and S21 were not signed in for the day and S12 failed to sign out for her break. The attendance log was corrected prior to the specialist leaving the center. Based on record review on 5/28/2026, at 12:30 p.m., S2 failed to have S24's CCCBC-based determination of eligibility for child care purposes from the Department available for review on the center’s CCCBC roster. S24's first day at the center was 2/16/2026. Corrective Action: Effective 6/8/2026, S1 stated S24 was added to the center's roster, and going forward she will verify that all staff are listed on the center's roster, to ensure compliance with this regulation.

Based on record review on 5/28/2026, at 12:30 p.m., S2 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the Department for S8 prior to them being present at the child care center or performing services. S8's date of hire and start date was 2/9/2026, and her CCCBC-eligibility date was 2/26/2026. Corrective Action: Effective 6/8/2026, S1 stated she will verify that all staff have an eligible CCCBC status prior to being present at the child care center or performing services, to ensure compliance with this regulation.

Based on interviews on 5/16/2025, at 11 a.m., children failed to be under supervision at all times. On 5/13/2025, S6 and S19 brought children, ages 1-2-years-old, inside from the playground and did not conduct a headcount. A parent arrived to pick up her children around 4:20 p.m., and saw a child playing in the side play yard. She parked her vehicle, went over to the fence, and saw it was her son, alone and unsupervised. She informed two staff, S6 and S19, that her child was alone on the playground. Based on observation at 11:20 a.m., child to staff ratio failed to be met for the following group of children: 6 infants with 1 staff. The required ratio for this group of children is 5 children per 1 staff person. This was corrected prior to the Specialist leaving the classroom. Corrective Action: Effective 4/25/2025, S1 stated she will retrain staff on ratio guidelines and notifying office staff when their classroom is at max or over child/staff ratio limits so proper accommodations can be made, to ensure compliance with this regulation.

Based on observation at 11:30 a.m., while supervising a group of 14 three-year-old children during nap time, S7 failed to devote her time to the supervision of the children as she was observed talking on her phone. This was corrected prior to the Specialist leaving the classroom. Corrective Action: Effective 4/25/2025, S1 stated she will retrain staff on supervision participation and reiterate that personal use of phones is prohibited while staff are in the classrooms, to ensure compliance with this regulation.

Based on record review at 12 p.m., S1 failed to have documentation that 1 of 20 staff on the premises and accessible to children had current certification in infant, child, and adult CPR through training approved by the Department. S18 failed to have current certification. A class has not been scheduled. S18 failed to have current certification. A class has not been scheduled. Corrective Action: Effective 4/25/2025, S1 stated she will schedule a Department-approved class for staff lacking current certification in PFA, and going forward will verify staff files regularly for all required certifications/documentation, to ensure compliance with this regulation.

Based on record review at 12:30 p.m., S1 failed to document that the entire center and play yard was checked after the last child departed to ensure that no child was left unattended at the center. S1 was unable to provide documentation that the end-of-day visual check was completed from 12/13/2024, until present. This was unable to be corrected during the inspection.

Based on record review at 9:30 a.m., S1 failed to document observations, when something is observed, noted on children upon arrival to the center. This was corrected during the inspection.
