      Based on record review at 10:45 a.m., the center's daily attendance record for children failed to accurately reflect the children on the child care premises at any given time. 9 children were present and 8 children were signed in on the log. S1 corrected the log by adding C7, 10-months-old, to the log prior to Specialist leaving the center. Corrective Action: Effective 5/21/2026, S1 stated she, or the floater, will double check the attendance log at 9:00 a.m. each day to ensure compliance with this regulation.

      Based on record review/interviews at approximately 1:30 p.m., S2 failed to ensure strings and cords were not accessible to children under age 4. An incident occured on 5/19/2026, at 2:55 p.m., were C1, 7-months-old, was accessible to the swing cord. He pulled the cord out of the wall socket and the head of the plug hit him in the forehead causing a red mark. At 1:40 p.m., during the inspection the cords to the traditional swing and the multi-motion swing were loose an accessible to children.       1103.A.5.&7.&C.1.&2.: Based on interviews on 12/15/2025, at 11 a.m., S1 failed to notify the Department and DCFS within 24 hours of the following critical incident: On 12/7/2025, S1 was made aware of an anonymous social media post alleging the center was administering Benadryl, aka: "pink nap juice", to children as a sleep aid without parental authorization. S1 reported the incident to the Department on 12/15/2025; however, DCFS failed to be notified. The specialist was unable to validate the allegation.
