      C:- Based on record review at 11:00 a.m., S3 failed to ensure within seven calendar days of the first day present at the center, and prior to assuming sole responsibility for any children, each staff member shall complete the LDE Key Training Modules 1,2, 3 and the DCFS online Mandated Reporter Training. The Specialist observed 2 out of 3 staff members did not complete the DCFS online Mandated Reporter Training. F.: Based on record review at 11:00 a.m., S3 failed to have CPR and First Aid Certification documentation for S1 and S2 within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children. The Specialist observed S1 (DOH:7/14/2025) and S2 (DOH :7/14/2025), supervising and assuming sole responsibility of 2 infant children. A class is schedule for 8-16-2025. Corrective Action: Effective 8-14-2025, S3 stated a class is schedule for 8-16-2025, to ensure compliance with his regulation.

      Based on record review at 11:15 a.m., S3 failed to provide documentation of End-of-Day-Check. Results including date and signature of Staff member completing the check not being documented. The Specialist observed that the End-of Day-Check had not been completed for the months of July 2025 and August 2025. The centers first day operating with children present was on 7/21/2025. Corrective Action: Effective 8-14-2025, S3 has printed out End-of Day document and placed on board to be completed daily, to ensure compliance with this regulation.
