Based on record review at 10:15 a.m., S1 failed to provide documentation of End-of-Day-Check. Results including date and signature of Staff member completing check has not been documented. The Specialist observed that the End-of Day-Check had not been completed for the month and days of 11/19/2025 to 11/25/2025. Corrective Action: Effective 12/5/2025, S1 stated she will create sign at the front door with End of Day Check paperwork, to ensure compliance with this regulation.

1901.J.&K.: Based on observation at 9:15 a.m., S1 failed to ensure items that can be harmful to children, were kept in a locked cabinet or other secure place that ensures they are inaccessible to children. The Specialist observed two heaters in a classroom: one small heater was placed on the floor, near a table of five, 1-3 year-old children and plugged in. Based on observation at 9:15 a.m., S1 failed to ensure personal belongings of center staff members were not accessible to children. The Specialist observed two purses on a table, a set of keys and a water bottle placed on a container of blocks. Corrective Action: Effective 12/5/2025, S1 stated she will put all personal items in vacant room, to ensure compliance with this regulation.

Based on record review, at 1:30 p.m., S1 failed to ensure all staff members obtained a minimum of 12 clock hours of continuing education per center anniversary year. No staff working at the center has obtained an official transcript to verify they have completed the 12 credit hours for the anniversary year. The Specialist observed a Mandated Reporter training certificate not offered through an approved agency. Based on the record review/interview of staff files on 06/30/2025 at 12:30 p.m., S1 failed to have documentation that no staff member, S1 (DOH 2/19/1999), completed the online child abuse and neglect Mandated Reporter Training provided by DCFS annually. The Specialist observed a training certificate not offered through an approved agency. Corrective Action: Effective 06/30/2025, S2 stated they will complete Mandated Reporter training through DCFS to ensure compliance with this regulation.

1513.A.1. 2.: Schedules- Based on observations at 9:45 a.m., S1 failed to post a schedule of days and hours of operation, including scheduled days and holidays when center is closed, a daily schedule must be posted in a location visible to both staff and parents. The Specialist observed that the center did not have a daily schedule posted in a visible location for parents and staff in the classroom. Corrective Action: Effective 6/2/2025, S1 stated she will create a schedule today, to ensure compliance with this regulation.

1727.A. Based on record review at 10:00 a.m., S1 failed to provide documentation that all staff completed the online child abuse and neglect Mandated Reporter Training provided by DCFS. The Specialist observed that 2 out of 3 staff had Mandated Reporter training. S3 did not have Mandated Reporter training. Corrective Action: Effective 6/2/2025, S1 stated S3 is completing training today, to ensure compliance with this regulation.

1915.A. Health Services - Observation: Based on record review at 10:15 a.m., S1 failed to provide documentation of observations being documented when something is observed, noted on children upon arrival to the center. Results including an explanation from parent and/or child were not documented. The Specialist observed that daily observations had not been completed by S2 on 6/2/2025. Corrective Action: Effective 6/2/2025, S1 stated upon arrival, each child will be observed and documentation would be completed, to ensure compliance with this regulation.

: Based on Observations, at 10:15 a.m., S1 failed to provide documentation of completed Tornado Drills for the months of March and April of 2025. Tornado drills shall be conducted at least once per month in the months of March, April, May, and June. Corrective Action: Effective 6/2/2025, S1 stated she will conduct all required tornado drills each year, to ensure compliance with this regulation.
