This Statement of Deficiencies was generated as a result of the on-site State licensure survey conducted at your facility on 01/12/2026. Please respond to each deficiency and attach documents as requested for the deficiency it pertains to. Sign and submit your Plan of Correction within 10 business days of receipt. The facility is licensed for 12 Children as a Group Care. The census at the time of the survey was 1 child and 1 staff. 1 children’s file was reviewed, and 1 staff file was reviewed. Reminders: If deficiencies are cited, an approved plan of correction must be returned within 10 days Based on inspection walk through, a hazard was observed in the bathroom. The crown molding/border in the bathroom was cracked with chipped paint, please resurface and repair and Upload photos of corrections and repairs to the Plan of Correction by 01/28/2026. Ensure that you answer all 4 questions in the POC box under "remarks." Based on file review during inspection, it was observed that Child #1 was missing their Health Statement. Please refer to child identifier list for name of child. Please request that child’s parent schedule requests a signed health statement for their child and returns to director for the child’s file. Upload a copy of this form into the Plan of Correction by 01/28/2026. Ensure that you answer all four questions in the POC box under the remarks section and fill in the “date POC submitted” box.
