      Based on observations and record review at 2:00 p.m., S1 failed to operate within the licensed capacity of the center as during the inspection the Specialist observed 38 children present on the premises, and the center is licensed for 36 children. Additionally, the Specialist observed 10 children, aged 8-weeks-old to 1-year-old, being supervised in a classroom which is currently unlicensed space. This could not be corrected prior to the Specialist's departure. Corrective Action: Effective 12/10/2025, S1 stated she will ensure she operates within the licensed capacity of the center…

      Based on record review at 2:00 p.m, S1 failed to ensure S2 completed the DCFS online Mandated Reporter Training within 7 calendar days of the first day present at the center. S2's first day present at the center was 11/17/2025, the training should have been completed by 11/24/2025. The training was not completed until 12/01/2025. Corrective Action: Effective 12/10/2025, S1 stated she will ensure all newly hired staff complete all new hire orientation trainings within the specified time frame to ensure compliance with this regulation.

      Based on observations, record review, interviews, at 3:00 p.m., S1 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the Department for S6. S6 was hired and began orientation at the center on 06/10/2025., and based on review of her sign-in sheet, she worked at the center from 06/10/2025 through 06/16/2025. S1 was not able to provide an eligible CCCBC prior to the Specialist's departure. S6 vacated the premises immediately.
