      Based on observation of the outdoor play yard, LPA observed the black tar like material peeling off and separating from the playground step in several areas. LPA also observed a broken undergound electrical box cover in the grass area in front of the play yard. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 02/27/2026
