The facility did not have documented proof that all child care personnel were trained and knowledgeable within 30 days of date of hire in [ safe sleep practices and shaken baby syndrome]. CCF Handbook, Section 4.2.3 and 4.2.5 (Section 2.1 Health and Safety, Page 3) Training [SR] There are currently 6 employees. All 6 employee records were reviewed. One employee, TA, did not complete the safe sleep training within 30 days of their hire date. The Child Care Facility Handbook states on page 41 that: 4.2.3 Safe Sleep/ Shaken Baby Syndrome Training All child care personnel who work in a facility that offers care to infants must have training regarding guidance on safe sleep practices, preventing shaken baby syndrome and abusive head trauma; recognition of signs and symptoms of shaken baby syndrome and abusive head trauma; strategies for coping with crying, fussing, or distraught child and the development and vulnerabilities of the brain in infancy in early childhood within 30 days of hire at the facility. For child care personnel to satisfy this requirement the training must be accomplished through one of following methods: Departments Health Safety and Nutrition course, Safe Sleep course, or the Early Learning Floridas Safe Sleep Practices. Documentation of training must be maintained on the Departments training transcript in the child care personnel record. Child care personnel did not complete preservice training requirements prior to unsupervised contact with or care of children or, if supervised, within 90 days of beginning employment or service with the provider. (Section 3.1 and 3.2 Training Requirements, Page 7) [SR] There are currently 6 employees. One employee, RA, did not complete the required SR preservice training within the 90-day time frame. New hires and current preservice trainees must complete all required preservice training within 90 days of initial employment with any SR provider by successfully completing one of the options below. If a new hire or preservice trainee elects to complete DCF courses in Option 1, they must also complete the two new health and safety courses (Health and Nutrition in the School Readiness Program and Safety Practices in the School Readiness Program). CCF Handbook, Section 7.2 (Section 19.2, number 1) Record Keeping [SR] There are currently 38 children enrolled. Counselor reviewed all 38 children's records. Four children had an expired physical. The Child Care Facility Handbook states on page 60 that: Child Care Facility Handbook Page | 60 documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations. C. School-aged children attending public or non-public schools are not required to have student health examination and immunization records on file at the child care facility as such records are on file at the school where the child is enrolled. D. If the custodial parents or legal guardians need assistance concerning these requirements, the facility shall refer them to the Department of Health or to the childs physician. E. Medical records in this section are the property of the custodial parent or legal guardian and must be returned to them when the child withdraws from the facility. The medical records are transferable if the child attends another facility. F.
