      The facility did not have documentation to show child care personnel had begun the introductory training within 90 days of employment in the child care industry. CCF Handbook, Section 4.1 Employee listed on the supplemental page did not start the DCF training within 90 days of employment. Employee has since started the training. Employee should have started the training by May 2025 and instead did not start the training until 08/12/2025. Technical Assistance given from the handbook: 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry.       Child Care personnel, who were not in compliance with training requirements when they left the industry, did not complete required training and any new mandated training before returning to the industry. That employee has worked in the industry before and had run out of time to complete the required 40 hours. Provider has suspended employee until training has been completed. CCF Handbook, Section 2.5.2 General Requirements [SR] Provider did not have a 2nd signature on the van log upon returning from drop off in the mornings. Technical Assistance given from handbook: 2.5.2 Transportation Log A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). Corrective Action: Provider must submit a week's worth of transportation logs showing a second signature.
