      1723.C. Based on observations at 10:20 a.m., S1 failed to have documentation that 2 of 10 staff on premises and accessible to the children have current certification in pediatric first aid through training approved by the Department. S5 and S6 only have Basic Life Support certification. Corrective Action: Effective 05/07/26, S1 stated she will ensure all new staff complete CPR/PFA certification approved by the department prior to assuming sole responsibility for any children to ensure compliance with this regulation.

      1723.F Based on record review and observations at 9:15 a.m., S1 failed to ensure S7(DOH 04/20/2026), had current CPR/PFA certification approved by the Department prior to assuming sole responsibility for any children. The Specialist observed S7 supervising 6 children, age 2-years old. This was not corrected prior to the Specialist departure.
