Based on record review and interviews on 10/28/2025, at 11:00 a.m., S1 failed to notify LDOE and DCFS within 24 hours of the following critical incident: On 10/15/2025, the parent notified S3 that C1, age 2-years-old, swallowed rocks from the center's play yard, which resulted in medical attention being needed. Corrective Action: Effective 11/12/2025, S1 stated she will will submit all critical incidents to LDOE and DCFS within 24 hours to ensure compliance with this regulation.
