Based on record review/interviews at 2:15 p.m., S1 failed to ensure children were under supervision at all times. On 12/12/2024 at 9:02 a.m., C1, age two-years-old, was left alone in the restroom by S6 and S23 for 3 minutes. S12 found C1 in the restroom at 9:05 a.m. and brought C1 to S6.
