C.: Based on record review and interview at approximately 11:40 a.m., S7 failed to ensure children were supervised at all times. According to the center's video footage dated 5/05/2026, C1, 1-year-old, was unsupervised from approximately 8:03 a.m. to 8:05 a.m. C1 exited to the right of the center's baby playground via a gate that the latch was not engaged properly. He ran down the center's driveway and turned right entering the grassy area that connects to the sidewalk, which runs along Valley View Blvd and the back of the center's playgrounds.
