Based on record review, observation and interviews, S28 (07/22/2024) failed to ensure children were supervised at all times. On 10/24/2024, C1, 4- years-old, was left unsupervised in the classroom from 1:58 p.m. until 2:06 p.m. S28 exited the classroom with the class at 1:58 p.m., but C1 was not with them, as she was sitting at a table with her head down in the classroom. S8 entered the classroom at 2:06 p.m. and saw C1 sitting at the table unharmed. C1 was left unsupervised for approximately 8 minutes. S28 was placed on suspension until further notice from management.

S23 returned C1 to his classroom at approximately 2:17 p.m.

S23 returned C1 to the classroom at approximately 2:17 p.m. C1's whereabouts were unknown for approximately 2 minutes. S33 was placed on leave until further notice.
