Based on interviews and record review on 05/11/2026, although the center implemented a Behavior Management Policy prohibiting physical punishment, S12 (DOH 08/14/2024) used physical punishment on C1, 4-years-old, on 04/22/2026, at approximately 9:00 a.m. S1 observed video footage of C1 using her right foot to kick S12's right leg shin, then S12 kicked C1 back using her right foot. On 02/05/2026, S14 went in the classroom restroom during naptime with C1, -3 years-old, then closed the bathroom door leaving 11 children unsupervised for approximately 1 minute. C.: Based on interviews on 3/31/2025, at 10:00 a.m., S7 failed to notify the Department and Child Welfare within 24 hours of the following critical incident: On 2/26/2025, at around 11:20 a.m., C1, 4-years-old, attempted to inappropriately touch C2, 3-years-old, while playing on the slide on the playground. At around 11:25 a.m., while sitting on the carpet in the classroom, C1 showed his private part to another child. S7 did not report the incident to S1.
