      Based on observations, record review, and interview at 10:00 a.m., S1 failed to ensure 1 of 14 staff members on the premises of the child care center and accessible to children had a current certification in adult and child CPR through training approved by the department. S9's CPR expired 3/31/2025. She is scheduled for CPR training on 4/10/2025. Corrective Action: Effective 4/7/2025, S1 stated she will keep a log that includes all staff's CPR expiration dates and will make sure they are recertified before they expire to ensure compliance with this regulation.

      Based on observations, record review, and interview at 10:00 a.m., S1 failed to ensure 1 of 14 staff members on the premises of the child care center and accessible to children had a current PFA certification through training approved by the department. S9's PFA expired 3/31/2025. She is scheduled for PFA training on 4/10/2025.       1103.A. Based on interviews at 10:00 a.m., S15 failed to notify the parent of the following critical incident: On 10/3/2024, C1, 3-years-old, was separated from her classroom while walking to the playground. She was left unsupervised for approximately one minute when O1 found her and brought her back inside the center. Contact was not made until 12:10 p.m. This incident could not be corrected prior to the Specialist departure.

      Based on record review/interviews at 12:00 p.m., S1, S4, and S19 failed to ensure children were supervise at all times. On 10/3/2024, at 9:38 a.m., S1, S4 and S19 were responsible for supervising 16 children, all 3-years-old, from their classroom to the playground when C1 was separated from the group. C1 was at the end of the line when the door closed behind S1 leaving her stranded between buildings 1 and 2. Per 15s verbal statement, C1 was found by O1 and brought back inside of the building at 9:39 a.m. This incident could not be corrected prior to the Specialist departure.
