      Based on record review, at 11:15 a.m., S1 failed to ensure S4 has a current certification in pediatric first aid and CPR through training approved by the Department within 90 calendar days from the date of hire on 10/20/2026. A class is scheduled for 3/16/2026. Corrective Action: Effective 3/11/2026, S1 will have all new staff to complete training prior to the end of their 90 day period to ensure compliance with this regulation.

      Based on record review at 11:30 a.m., S2 failed to immediately notify the parent of C1, age 3 years old, of the following incident: on 11/07/205, at 9:31 a.m., S6, teacher, and S7, Paraprofessional, left C1 unsupervised on the playground for approximately three minutes. S2 notified the parent at 10:26 a.m., after informing S1of the incident. Corrective Action: Effective 11/19/2025, S2 stated she will allow member of management to notify a parent immediately if or when she is unavailable to do so to ensure compliance with this regulation.

      1713.A.&B.&C.: Based on record review/interview at 12:30 p.m., S6 and S7 failed to supervise C1, 3-years-old, at all times. On 11/07/2025, at 9:31 a.m., S6 and S7 left C1 on the playground. At an unknown time, C1 was returned inside the building by O3. O3 observed C1 waving at the door. O3 opened the door and alerted S2. C1 was returned to the classroom with S6 and S7 by S2 at an unknown time. Corrective Action: Effective 11/19/2025, S2 stated she will encourage staff to stop and count when arriving and exiting the playground and classroom to ensure compliance with this regulation.
