      Based on record review at 11 a.m., the center's staff and owner's daily attendance record failed to accurately reflect persons on the child care premises at any given time. S1 was observed present at the center but failed to be signed in. This issue was corrected prior to the Specialist exiting the center.

      Based on record review at 11 a.m., S1 failed to have documentation that 2 of 25 staff, S16 and S25, completed orientation requirements within seven and thirty days of the first day present at the center and prior to having sole responsibility for any children. S16 (DOH 9/23/2024), failed to complete the DCFS Mandated Reporter Training within seven days of her first day present and LDE Key Training Module 3 within thirty days of the first day present at the center.       Based on observation/record review at 10:35 a.m., information regarding a food allergy of C2, two-year-old, failed to be posted in the food preparation area.
