      1711. A.B.D.: Based on observation at 9:45 a.m., S1 failed to meet the required child-to-staff ratio for children of the following ages: 6, infants, with S13. The required ratio for children of this age is 5 children per 1 staff person. The ratio was corrected when an infant was transitioned to S14 and S15's classroom. Corrective Action: Effective 06/11/2026, S1 stated she will review the ratio with all new staff, to ensure compliance with this regulation.

      1919. H. Based on interviews on 5/27/2026 at approximately 3:00 p.m., S15 propped a bottle on a blanket to feed C1, three-months-old, while seated in a floor swing. Corrective Action: Effective 06/11/2026, S1 stated S15 was transferred to the older infant classroom, an additional staff member will be placed in the classroom, and staff will be retrained on all infant policies, to ensure compliance with this regulation.

The Specialists observed C1, seven-months-old, and C2, three-months-old, asleep in floor swing. S11 corrected and placed the infants in crib. Corrective Action: Effective 01/27/2026, S2 stated she will retrain all staff on Safe Sleep practices by 01/30/2026, to ensure compliance with this regulation.
