      Based on observations at 9:30 a.m., S1 failed to ensure moveable equipment was secured and supported so that it will not fall or tip over. The Specialist observed plastic pieces, play yard equipment, detached lying on the ground and accessible to children. Corrective Action: Effective 6-11-2026, S1 stated equipment pieces will be removed from the play area, to ensure compliance with this regulation.

      Based on observations at 9 a.m., S1 failed to ensure all indoor hazards were inaccessible to children. The Specialist observed a standing oscillating fan in the entrance hallway of the center. The fan was accessible to children. Corrective Action: Effective 6-11-2026, S1 stated a fan would no longer be in use and will be placed into the Director's office, to ensure compliance with this regulation.

      1723.A.&B.:Based on record review/interview at 10:15 a.m., S1 failed to have documentation that 1 out of 6 staff members have current certification in infant, child, and adult CPR through training approved by the Department. S2 (DOH: 10-2-2023), failed to have the infant, child, and adult CPR through training approved by the Department. Corrective Action: Effective 10-22-2025, S1 stated she is currently enrolling S2 in a class to be completed before 10-30-2025 , to ensure compliance with this regulation.

      1723.C Based on record review/interview at 10:15 a.m., S1 failed to provide documentation that all staff on the premises and accessible to the children have current certification in pediatric first aid through training approved by the Department. The Specialist observed that 1 out of 6 staff members, S2 (DOH: 10-2-2023), did not have current certification in Pediatric First Aid through training approved by the Department. Corrective Action: Effective 10-22-2025, S1 stated she is currently enrolling S2 in a class to be completed before 10-30-2025 , to ensure compliance with this regulation.

      Based on observations/ Interview, S2 allowed a bib to be worn by an infant while asleep.The Specialist observed one four-month-old infant in the crib asleep with a bib around her neck. Corrective Action: Effective 10-22-2025, S1 stated she will meet with staff to discuss regulations of the infant room and will post reminders in the infant room in regards infant room policy, to ensure compliance with this regulation.

      1727.A. Based on record review at 1:00 p.m., S1 failed to provide documentation that all staff completed the annual online child abuse and neglect Mandated Reporter Training provided by DCFS. The Specialist observed that 1 out of 3 staff did not have Mandated Reporter training. S2 did not have Mandated Reporter training. S2s Mandated Reporter training expired on March 12, 2025. Corrective Action: Effective 9/4/2025, S1 assisted S2 in logging in to complete mandated reporter training, prior to specialist departure, to ensure compliance with this regulation.

      1901. C.: End-of-Day Check-Based on record review at 10:45 a.m., S1 failed to provide documentation of End-of-Day-Check. Results including date and signature of Staff member completing the check not being documented. The Specialist observed that the End-of Day-Check had not been completed for the month and days of 6-13-2025, 7-17-2025, 8-4-2025, 8-6-2025, 8-7-2025, and 8-8-2025. Corrective Action: Effective 8-11-2025, S1 stated she will speak with staff regarding policies and procedures for End-of Day Check, to ensure compliance with this regulation.
