C.: Based on record review and interview at 1:00 p.m., S1 failed to ensure 3 of 7 staff obtained the required minimum clock hours of continuing education per center anniversary year. S4 had 1.5 hours of the required 3 hours. S5 had 9.5 hours of the required 12 hours. S6 had 5.5 hours of the required 12 hours. This could not be corrected. Corrective Action: Effective 9/8/2025, S1 stated she will check staff's progress every February and will give staff an additional 3 days to complete all trainings to ensure compliance with this regulation.

Based on observations, record review and interview at 10:30 AM, S1 failed to ensure the daily attendance log for children included name of the child, the time of arrival and departure of each child and the name of the person to whom the child was released. 11 children were present at the center, but only 10 were signed in. C1, 2-years-old , was present but not signed in on 8/8/2024. S1 stated his grandmother brought him and forgot to sign him in. Based on observations, record review, and interview at 10:30 AM, S1 failed to ensure documentation of a CCCBC-based determination of eligibility (CCCBC) was available for 1 of 5 staff, prior to the individual being present in the childcare facility. S5's - date of hire was 5/20/2024 according to her time sheet and file, but her CCCBC is dated 5/22/2024. She was present from 5/20/2024-5/21/2024. Documentation of a CCCBC-based determination of eligibility (CCCBC) was not conducted for S5 at least once during a five-year period.

Based on record review and interview at 10:30 AM, S1 failed to ensure the center had a current annual inspection and approval from State Fire Marshal. The date of the last approval is 6/15/2023. An inspector came out on 5/22/2024, but due to the fire extinguished not being inspected, he did not issue a current OSFM report. S1 states she has called multiple times to get an inspector to come out since the fire extinguisher has been inspected, but no one has come.
