Based on observations/interviews at 12:15 p.m., Children were not under supervision at all times as S5 and S6 failed to know how many children were in their care at all times. The Specialists observed 30, 1-to-5 year old children, sleeping in the classroom during the walkthrough, but S5 and S6 stated they had 14 children. Corrective Action: Effective 11/17/2025, S1 stated she will complete daily walkthroughs to ensure staff are properly monitoring assigned children and maintain count to ensure compliance with this regulation.

1713.E., Based on observation at 3:21 p.m., while supervising a group of 5, 1-year-old children, S7 failed to devote her time to supervision of the children, meeting the needs of the children, and in participation with the children in their activities. The Specialists observed S7 sitting on the table and looking down to her cellphone during class time while the children sat on the rug and walked around the classroom. Corrective Action: Effective 11/17/2025, S1 stated she will complete daily walkthroughs of the classrooms to ensure compliance with this regulation.

S2 is the only staff with documentation of the of the current training. Corrective Action: Effective 11/17/2025, S1 stated she will have staff complete the training yearly to ensure compliance with this regulation.

Based on record review at 1:30 p.m., S1 failed to have documentation that 1 of 7 staff completed the required annual Online Child Abuse and Neglect Mandated Reporter Training provided by DCFS. S3âs mandated reporter certificate expired on July 18, 2025. Corrective Action: Effective 11/17/2025, S1 stated she will set a reminder 30 days prior to certificate expiration to ensure compliance with this regulation.

The Specialists observed C1 sleeping in a high chair in the infant classroom. Corrective Action: Effective 11/17/2025, S1 stated children will be transitioned to their assigned crib immediately when they fall asleep to ensure compliance with this regulation.

1909. G. Based on observations at 12: 07 p.m., S1 failed to ensure bibs arenât worn while sleeping. The specialists observed C1, age four-months- old, sleeping in a high chair while wearing a bib. S3 removed the bib from the infant during the licensing visit. Corrective Action: Effective 11/17/2025, S1 stated she will remind staff to remove bibs when infants fall asleep to ensure compliance with this regulation.

1911.G. Based on observations at 12:08 p.m.,S1 failed to ensure pacifiers were not attached to the infants clothing. The Specialists observed a pacifier attached, by a clip, to the clothing of C1, age four-months-old. S3 removed the clip during the licensing visit. Corrective Action: Effective 11/17/2025, S1 stated she will remind staff to not clip pacifiers to infants clothing to ensure compliance with this regulation.
