Based on observations, record review and interview at 1:30 p.m., S1 failed to ensure documentation of a CCCBC-based determination of eligibility (CCCBC) was available for 1 of 9 staff, S9, date of hire 3/24/2026, prior to the individual being present in the childcare facility. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 4/1/2026, S1 stated before any staff starts their new position, they have received documentation of an eligible CCCBC and to ensure compliance with this regulation.

S9's first day at the center was on 9/3/2025. An eligible CCCBC was not obtained until 9/9/2025. S10's first day at center was on 9/3/2025. An eligible CCCBC was not obtained until 9/16//2025.       Based on observations, record review, and interview at 2:00 p.m., S1 failed to maintain a daily attendance record for staff that included arrival and/or departure times. S6 (DOH 6/3/2024) failed to sign out on 8/23/2024 and 8/27/2024. S8 (DOH 3/1/2021) and S9 (DOH 4/5/2022), floaters from another location, both failed to sign out on 8/22/2024. The attendance records were not corrected while the Specialist was present.
