Based on record review, observation, and interview at 10:30 a.m., S1 failed to ensure the children were supervised at all times. On 5/4/2026, at 9:59 a.m., C1, 2-years-old, entered the center alone. S1 went inside the center at 10:00 a.m., and brought C1 back outside with the rest of the class. Corrective Action: Effective 5/19/2026, S1 stated the doors are now being locked from inside to out to prevent the children from opening the doors and transitioning alone. They are redirecting the children away from the door to the playground to ensure compliance with this regulation.

S6 last certificate is dated January 2023. This could not be corrected prior to the Specialist's departure.
