S23 and S30 failed to have documentation of the certification. S1 stated both staff took the training, but she does not have a copy of the certificate. Corrective Action: Effective 03/30/2026, S1 stated she will ensure to get documentation of the training certificates timely to ensure compliance with this regulation.
