Based on record review and interview at 10 a.m., S1 failed to notify the Department within 24 hours of the following critical incident: On 09/18/2025, at 9:47 a.m., C1, two-years-old, went to take a toy from C2, two-years-old, while he was sitting on the floor. Based on observations/interview at 12:00 p.m., on 7/21/2025, S2 failed to ensure a daily attendance record for all visitors was maintained at the center to include the name, date of visit, arrival and departure times, and the purpose of the visit. S2 stated O2, a representative from Child Welfare visited the center during the week of July 2025, but she could not confirm the specific date because she did not sign into the visitor's log. Based on observations at 12:00 p.m., on 7/21/2025, the center was not free of hazards as there were 12 outlets missing covers, which were located in multiple classrooms throughout the center. Based on record review/interview at 12:00 p.m., on 7/21/2025, S2 failed to ensure the physical condition of each child was observed for possible signs of illness, infections, bruises or injuries, and when something was observed, it was documented and such documentation included an explanation from the parent or child. Based on record review and interviews at 12:00 p.m., S1 and S2 failed to have documentation of an incident that occurred involving a head injury and failed to immediately notify the parent. On the morning of 6/30/2025, C1 (two-year-old) hit his head on the concrete play area floor outside several times, which resulted in a bruise on his forehead. S10 took him to S1 in the office to report the incident to O1. S1 took C1 to another classroom for the remainder of the day. Center's video showed on 06/02/2025, at 3:02 p.m., C3, one-year-old, grabbed a pair of scissors off of the edge of the counter. C4, one-year-old, then grabbed the scissors from C3. Based on record review/interview on 06/05/2025, at 10:30 a.m., S10, S13, and S19 failed to have items that can be harmful to children kept in a locked cabinet or other secure place that ensured they are inaccessible to children. On 06/02/2025, at 3:05 p.m., C1, two-year-old, was cut with a pair of scissors that was accessible to the children on a low countertop. Corrective Action: Effective 06/05/2025, S1 stated he will having a meeting with staff on 06/14/2025, to review items that can be harmful to children to ensure compliance with this regulation.

Based on record review/interview at 10:30 a.m., S16 (DOH: 04/08/2024), S22 (06/10/2024), and S24 (DOH 06/17/2024) failed to have current certification in pediatric first aid and CPR within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children.
