Based on record review and interview at 1:00 p.m., S1 failed to immediately notify the Parent of the following critical incident: On 6/16/2025, at 8:00 a.m., S35 informed S36 that she heard a slap noise and C1 (three-years-old) began crying in her classroom as S28 watched over her classroom when she stepped outside for a few minutes. S35 thought S28 slapped C1, but she did not see anything. S36 stated C1 informed her that S28 hit him, but S28 stated she did not. S28 stated C1 hit the floor making a slapping sound.
